**Responsibilities** **Physical Therapist - Community Hospital of San Bernardino** Join a compassionate and skilled rehabilitation team where your expertise as a Per Diem Physical Therapist will help patients regain function, independence, and confidence. At Community Hospital of San Bernardino, we are looking for a licensed professional to provide high-quality care across a range of medical conditions and patient populations. As a valued member of our therapy team, you will evaluate and treat patients using evidence-based practices, craft individualized treatment plans, and collaborate with interdisciplinary teams to deliver patient-centered care. This role allows you to make a meaningful difference while maintaining flexible scheduling. **Key Responsibilities Include:** + Perform comprehensive evaluations and establish personalized treatment goals + Administer therapeutic interventions in compliance with physician orders and professional standards + Educate patients and caregivers on home exercise programs and recovery expectations + Document patient progress accurately and timely in accordance with hospital policies + Participate in discharge planning and coordinate with care teams to ensure optimal outcomes + Provide guidance and oversight to support personnel and therapy students + Contribute to quality improvement efforts and department initiatives This position reflects an active commitment to the mission and values of Community Hospital of San Bernardino and supports our dedication to excellence in patient care. **Qualifications** + Current registration, certification, or license as a Registered Physical Therapist by the respective state of which one practices + BLS - Basic Life Support certification is required. + Bachelors (BA, BS) degree in Physical Therapy is required. **Overview** Founded in 1910, Dignity Health - Community Hospital of San Bernardino is a 347-bed, acute care, nonprofit, community hospital located in San Bernardino, California.
