Be a Champion for Patients: Providing Personalized and Compassionate Care
As a vital member of our team, you'll play a key role in delivering personalized care to patients, particularly those with chronic illnesses. You'll be a float resource, providing support in areas of rapid growth, making a real difference in expanding access to quality care. This role involves working primarily in a virtual environment, with occasional travel within a specific region for in-home assessments.
Your Mission: Empowering Patients to Live Healthier Lives
Conduct comprehensive health assessments, developing strategies to improve or manage patients' health and promote healthy habits.
Complete 3+ patient visits daily, both virtually and in-person (as needed).
Counsel and educate patients and their families about available benefits and programs.
Maintain accurate and timely patient records, including chief complaints, diagnoses, medical history, medications, allergies, assessments, and plans.
Schedule and administer follow-up appointments.
Ensure adherence to quality of care standards.
Assist patients in accessing educational resources.
Actively participate in integrated care team meetings.
Assess patients' health status, implement care plans, and follow up to evaluate their progress.
Complete all necessary documentation promptly.
Other duties as assigned.
Your Toolkit: Skills and Qualifications
Active and unrestricted Registered Nurse (RN) license.
Access to reliable transportation, a valid driver's license, and car insurance.
Proficiency with required medical instruments and equipment.
Knowledge of computer-based data management programs, information systems, medical records, and point-of-interview technology.
Excellent verbal and written communication skills.
Sound understanding of federal and state regulations, including HIPAA and OSHA.
2+ years of direct patient care experience.
